1.) Starting April 15 until July 31, 2013, for every P200.00 purchase of any Cotton
Club Underwear, customers are entitled to join to our promotion.
2.) Customers must fill up the raffle coupon with his / her name, address, contact
number (if any), E-mail address (if any), affix signature
3.) Coupons may drop in the designated drop box located in all participating
Department Stores nationwide.
4.) Prizes at stake:

5.) Raffle draw will be held at the main office of Cotton Club at unit 1501, State
Center Bldg., Juan Luna St., Binondo Manila.Raffle draw will be on August 14,
2013at 3:00 PM in the presence of a DTI representative.
6.) Customer can only win once during the promo period.
7.) Deadline for submission of entries is until closing time of the participating
stores on July 31, 2013.
8.) Sealed drop boxes shall be collected one (1) day after the final date for
dropping of entries.
9.) All winners will be notified through e-mail, telephone call, and registered
mail.List ofwinners will also be posted in all participating stores / outlets
10.) Prizes are non transferable and not convertible to cash. Registration of the
motorcycle shall be shouldered by Wellcome Global Trading Corp.
11.) Prizes are tax free.
12.) To claim prize, winner must present the notification letter, winning raffle stub
and 2 valid I.D. In the event that winners cannot present the raffle stub,
winners need to submit an affidavit of loss duly notarized.
13.) Winners can claim their prizes at Cotton Club Head Office at Unit 1501, State
Center Bldg., J. Luna St., Binondo Manila.
14.) For provincial winners, they can claim prize at our nearest satellite office as
identified in the registered mail.
15.) Unclaimed Prizes within 60 days from the date of receipt of notification of
winners will be forfeited in favor of Wellcome Global Trading Corp. with prior
DTI approval.
16.) All employees of Wellcome Global Trading Corp. and its subsidiaries and
sister companies including their relatives up to the 2nd degree of
consanguinity or affinity are not qualified to join the promotion.
