Promo Mechanics: 

1. Shoppers are entitled to one (1) raffle coupon for a chance to win a tablet, a media player or a Jockey® gift certificate.

 

2. To join, shoppers must fill up the raffle coupon with the following information: name, address, telephone number OR mobile number, email address and store branch. Shoppers must affix their signature and drop 1. For every single receipt purchase worth P500.00 of JOCKEY® products at any participating store, their raffle entries at designated JOCKEY® drop boxes provided at all participating stores. Raffle entries must only be dropped in drop boxes located in the same store where purchase was made.

 

3. Raffle entries must be dropped in drop boxes until store closing hours of March 31, 2013.

    

4. Prizes are the following:

1st prize:6 winners of 1 (one) unit iPad Mini Wifi 16 gig. Dimensions: Height: 7.87 inches (200 mm) Width: 5.3 inches (134.7 mm) Depth: 0.28 inch (7.2 mm) Weight: 0.68 pound (308 g)

5. Raffle draws will be witnessed by a DTI representative and shall be conducted at GTVL Manufacturing Industries Inc. on April 14, 2013 at 5:00pm.

 

6. Participants can only win once. In the event a participant is drawn more than once, he/ she gets the the prize of higher value.

 

7. Winners will be notified thru registered mail, phone call, email and sms (if mobile number was provided).

 

8. Winners may claim their prizes from Monday to Friday, 10 AM to 5 PM at the nearest participating branch as identified in the registered mail. Winners must bring with him/her the winning coupon and a valid identification card (SSS, Driver's License, Passport, etc.).

 

9. When claiming through a representative, requirements are: a.) Valid ID of authorized representative b.) Authorization letter and c.) All requirements in # 8. In case of lost coupon, the following must be presented: a.) Notarized affidavit of loss b.) Valid ID and c.) Notification Letter.

 

10. Redemption period shall be within 60 days upon receipt notification; otherwise prize shall be forfeited in favor of GTVL with prior DTI approval.

 

11. Major and minor prizes are not convertible to cash and 20% tax for the prize exceeding P10, 000.00 will be shouldered by the winners.

 

12. Non-exclusive: Shoppers may avail of this promotion together with other promotions.

 

13. This promotion runs from January 1, 2013 to March 31, 2013.

 

14. Employees of GTVL and participating branches including their relatives up to the second degree of consanguinity or affinity are disqualified from joining this promotion.
